- A copy of the expired passport or identity card
- A copy of the full family book
- Photograph
- Written letter with official supporting documents stating reasons for staying out of the country

- A complete copy of the family book
- A copy of the lost passport
- A copy of the report of the loss of the passport certified by the Ministry of Foreign Affairs of the country of residence and the country's mission abroad
Documents required by virtue of the damage of the regular passport
- A copy of the passport or Emirates ID card
- A complete copy of the family book
- A written letter stating the reasons for the damage of the passport, or a police report
- The damaged passport and handing it over to the mission upon coming to the embassy
